The policy also provides clearer decision-making authority for sanitarians, including discretion in several areas, and
includes language revisions to reduce redundancy.

Pre-inspection procedures were updated to specify supervisor-verified equipment maintenance, documentation of
corrective actions in EH Manager, and a reminder that unresolved violations must be reviewed during inspection
preparation. Additional operational reminders include washing thermometers in front of operators and clarification that
certain equipment may be grandfathered for up to five years before updates are required.

Written communication requirements were revised to reference applicable ordinances. Enforcement language was
updated to clarify how compliance dates are assigned based on priority, how repeated violations are documented, and
when follow-up actions or administrative conferences may occur at the sanitarian’s discretion.

Staffing updates noted an expansion of partner agencies available to assist with coverage. The Board also discussed the
increasing number of VRBOs and the desire to evaluate if Stevens, Traverse, and Grant could be added to Horizon’s
Food, Pool, Lodging delegation from MDH.

Motion by Shane Schmidt, seconded by Jerry Rapp to approve the Inspection and Enforcement Policy and Procedure
updates as presented. Motion carried unanimously.

Well Water Protection Grant Award:

Horizon has been awarded a new Well Water Protection Grant in the amount of $273,420, with a grant period from late
2026 through May 1, 2029. The grant’s objective is to test 10% of private wells within the Horizon service area for five
common contaminants.

Horizon has developed significant experience with well-water testing through the previous grant. This new grant is
focused specifically on testing, with testing provided at no cost to residents across all five counties.

The grant includes funding for a 0.5 FTE staff position to assist with client communication and conversations related to
the testing program. Horizon will complete expenditures under the current grant before transitioning to the new grant
funding.

The Communication Coordinator will assist with promoting the program and increasing public awareness through press
releases, newspaper and radio advertising, and other communication efforts.

Health Equity Policy Update:

An updated Health Equity Policy was presented to the Board for approval. The revisions strengthen Horizon Public
Health’s commitment to integrating health equity into organizational policies, programs, services, communications,
resource allocation, workforce practices, partnerships, and decision-making.

The updated policy clarifies responsibilities for leadership and staff, incorporates health equity into planning and quality
improvement, and outlines expectations related to community engagement, data and assessment, communication and
accessibility, workforce practices, resource allocation, and partnerships. The policy also emphasizes using community
input and data to identify and address barriers and inequities and to guide ongoing program and service improvements.

Motion by Marcia Schroeder, seconded by Bob Kopitzke to approve the Health Equity Policy updates as presented.
Motion carried unanimously.

Toward Zero Deaths Grant Applications: Resolution 26-16 and 26-17:

Two Toward Zero Deaths (TZD) grant applications through the Minnesota Office of Traffic Safety were presented for
Board approval. The Board approved motions for Resolution 26-16, serving Douglas County, and Resolution 26-17, a
joint application serving Grant and Pope Counties. Both grants focus on reducing serious injury and fatal crashes through
data-informed prevention activities, community engagement, and quarterly coalition meetings involving the four E’s:
Engineering, EMS, Enforcement, and Education.

The Douglas County application requests $24,000 in funding, with a $14,000 match/in-kind requirement. Douglas County
has a long-standing TZD project, with data showing a reduction in serious injury and fatal crashes.

Motion by Larry Lindor, seconded by Dwight Nelson to approve the Douglas County Toward Zero Deaths Grant
Application - Resolution 26-16 as presented. Motion carried unanimously.

The joint Grant/Pope County application requests $18,500, with a $10,600 match/in-kind requirement. The application
will focus on bringing Pope County into a collaborative TZD effort with Grant County, where serious injury and fatal
crashes have increased in recent years.

Motion by Gordy Wagner, seconded by Dwight Walvatne to approve the joint Grant/Pope County Toward Zero Deaths
Grant Application - Resolution 26-17 as presented. Motion carried unanimously.

It was noted that Stevens/Traverse Counties currently represent a gap in TZD coverage; however, available data does
not currently demonstrate sufficient need to support an application. Future consideration would include available data,
coalition development, staffing, and other program capacity.

HPH Strategic Plan annual progress review:
A review of the 2023-2026 Strategic Plan was presented to the Board. The plan runs from 2023 through 2026, and
Horizon Public Health successfully completed all strategies and met all goals outlined in the plan.

The work accomplished through the current strategic plan will continue to be carried forward and incorporated into the
next strategic plan.

Strategic Plan Consultant Contract:
A request was presented to the Board for approval of a contract for an HPH Strategic Plan Consultant. The current

strategic plan covers 2023-2026, with the planning process for the 2027 strategic plan expected to begin this fall.

Board Community Member Representatives Ms. Schroeder and Ms. Molesworth will participate in the planning process.
Laine Resources will facilitate the strategic planning process in accordance with Public Health Accreditation Board
(PHAB) requirements.

The proposed contract is for $5,000 and will be funded through CDC Infrastructure Grant funds.

Motion by Bob Kopitzke, seconded by Shane Schmidt to approve the Strategic Plan Consultant Contract as presented.
Motion carried unanimously.

HPH Workforce Development Plan annual progress review:
The annual progress review of the HPH Workforce Development Plan was presented to the Board. The plan covers

2023-2027.
Health Equity Goal #1 has been completed.

Priority #2: Performance Management and Quality Improvement continues to be addressed. The goal is that, by
December 2027, all HPH programs will proactively identify at least one task or duty within their workflow that adds
minimal value and implement a modified process.

Priority #3: Continuous Improvement and Engagement also continues to be addressed. The objective is that, by
December 2027, HPH will support a culture of learning in which every staff member has the opportunity to complete at
least one training or strengthen a skill through learning something new or enhancing an existing skill, supporting both
professional growth and organizational goals.

Anew plan will be created beginning in 2028.

Annual Reaccreditation and PHAB Site Visit Update:

An update on Horizon Public Health’s reaccreditation process through the Public Health Accreditation Board (PHAB) was
presented to the Board. HPH received its initial PHAB accreditation in March 2021, with reaccreditation required every
five years.

HPH submitted documentation for reaccreditation covering 117 measures in January 2026 and responded to initial
review questions in May 2026. A conflict-of-interest form was submitted in July 2026, followed by a Community Context
Session with PHAB on August 25, 2026.

The PHAB site visit is scheduled for October 19, 2026. Board members serving on the PMC were invited to participate in
the site visit.

MDH Regional Data Hub Grant Application:

A request for Board approval was presented for a five-year Regional Data Hub Grant application through the Minnesota
Department of Health (MDH). The grant period would run from January 1, 2027, through December 31, 2031, with
$200,000 in ongoing funding available each year and no required match or in-kind contribution. The application is due at
the end of September.

The grant requires regional collaboration and would serve the West Central SCHSAC Region, including Douglas, Grant,
Pope, Stevens, Traverse, Clay, Becker, Otter Tail, and Wilkin Counties. Participating counties may choose to be active or
inactive partners; all counties have elected to participate as active partners.

The proposed project would build a shared regional approach to public health data capacity. Key activities include
establishing regional priorities, assessing shared data needs and capacity gaps, hiring shared data expertise such as an
epidemiologist or data scientist, developing data infrastructure, providing analysis and technical assistance, completing
priority data projects, and evaluating the impact of the work for continuous improvement.

Motion by Larry Lindor, seconded by Gordy Wagner to approve the five-year Regional Data Hub Grant application as
presented. Motion carried unanimously.

CHB Meeting Format Options:

A request was brought forward for the full Board to consider offering a hybrid option for Community Health Board (CHB)
meetings. The Executive Committee previously reviewed the meeting format on March 11, 2024, and May 12, 2025, and
on both occasions recommended not offering a hybrid option. -

An update to the Minnesota Open Meeting Law, effective May 24, 2025, now allows CHB members to participate
remotely through interactive technology while maintaining a regular physical meeting location. At least one CHB
member must be physically present at the regular meeting location, and remote members may participate fully, count
toward quorum, and vote. The technology must allow members and the public to see and hear the meeting, and all
votes must be conducted by roll call. Meeting notices and minutes must also identify the use of interactive technology.

Board discussion included the potential benefits of allowing remote participation, such as providing an option for

members who are ill, as well as concerns that a hybrid option could reduce in-person attendance or participation. The
impact of roll-call voting on meeting length was also discussed. The item will be brought back for further discussion at
the next meeting with additional information, including clarification regarding participation and voting requirements.

Personnel Update:
The newest Sanitarian has been approved by the Minnesota Department of Health (MDH) to sit for the Registered
Sanitarian (RS) exam.

A new Hospice Supervisor started on August 31. Recruitment continues for casual RNs and Home Health Aides to
support Hospice services.

Horizon Public Health is also collaborating with the University of Minnesota Morris on internship pathways for students
in its Public Health Certificate program.
